Purpose

To file an internal complaint regarding procedural irregularities, conduct concerns, or compliance failures involving Council personnel. Form IA-3 is the standard intake instrument of the Office of Internal Affairs for filings originating within the Council’s own staff. It is the residual and primary filing for Council personnel reporting on Council personnel; not addressed by Bureau-level review and not by external complaint processes.


Filing Instructions

  1. Who files. Any Council license-holder.
  2. When. No later than ninety (90) days from the filer’s awareness of the matter complained of.
  3. Copies. One, sealed. The filing is routed by the filer’s Bureau Director to OIA. No routing party is authorized to retain, suspend, or amend.
  4. Where. Sealed pouch to the filer’s Bureau Director’s office. The Bureau Director forwards to OIA central within fourteen (14) days.
  5. Fees. None.
